3. Identification and exact URL of the material to be removed or disabled.
4. Your name, mailing address, telephone number and email address.
5. A statement that you have a good-faith belief the disputed use is not authorised by the owner, its agent or the law.
6. A statement, under penalty of perjury, that the notice is accurate and that you are the owner or authorised to act for the owner.

We review notices, may request clarification, and may remove or disable access to material when appropriate. We may notify the affected contributor and retain records needed for legal compliance. Repeat infringers may lose access to the service.

5. Counter-notification

A person who believes material was removed by mistake may send a counter-notice containing identification of the removed material and its former location, contact details, consent to the appropriate court's jurisdiction, acceptance of service of process, a statement under penalty of perjury that removal resulted from mistake or misidentification, and a signature.

6. Review and action

We review complete notices promptly and may remove or restrict disputed material while a claim is assessed. We may contact the submitter or content provider when additional information is required.
